Complete the documents located inside the Accident Packet in (that should be in each vehicle) and submit those forms to your Supervisor within 1 business day 7. Address Immediate Safety Concerns – Disengage but leave Equipment in place 2. Document Area – Take photos of equipment location, site locates, and any visible damage – Take Video footage of accident area including equipment location, site locates, and any visible damage 3. Notifications – Call Supervisor to report the accident – Contact Utility that incurred damage 4. Acquire Contact information of all parties involved – First & Last Name – Company / Entity Affiliation – Phone Number – Email Address 5. Submit an Internal Strike Report
